Red Sucker Lake Health Authority is a facility providing services in the Nursing Station as well as, mental health, diabetes education, and Home and Community care and operates as a part of the Four Arrows Regional Health Authority.

Red Sucker Lake Reserve No. 1976 is located on a peninsula on the northeast shore of Red Sucker Lake close to the Ontario border, approximately 709 kilometres northeast of Winnipeg.

About the opportunity 

Red Sucker Lake Health Authority has an exciting opportunity for a Nursing Case Manager to join our team. In this pivotal role, you will be an integral part of getting our new program called Jordan's Principle (which will work with children with disabilities while including the family in the process) off the ground. 

Responsibilities include, but are not limited to:

  • Assisting children with disabilities live as fully and independently as possible with family support
  • Developing service plans and working on program development
  • Writing progress reports for families participating in programming as well as reports to the Health Director and funding agencies as requested/required
  • Performing home visits and coordinating respite services
  • Taking direction from the Health Director as required
  • Gathering relevant information from parents, families, educators, and medical providers as it pertains to clients
  • Maintaining confidentiality
  • Coordinating, monitoring, and maintaining all program client appointments as pertains to the program
  • Following-up with health care providers and clients to ensure a clear understanding of results (outcomes)
  • Managing 8-10 staff members (scheduling, hours of work, etc.) and coordinating training for staff
  • Performing general office administration
  • Scanning, uploading, faxing, photocopying, inputting of documents as required

In order to qualify for this role, you will be an RN, or LPN and be certified in disability management. You must also have previous supervisory, case management, and clinical assessment experience. Preference will be given to candidates with a background in rehab care. Please note you must be in good standing with CRNM/CLPNM

Above all, you must be a self-motivated and team-oriented problem solver who has the ability to take ownership of your work. Previous experience with First Nations communities will be highly regarded, and experience working in paediatrics and/or disabilities is preferred

You must also have a valid driver's license and a have a clear criminal record check, child abuse registry check and drivers abstract.
